I understand that submission of this application does not guarantee service availability or immediate activation of services. Service availability, installation scheduling, equipment requirements, deposits, and account approval remain subject to NDTC review and applicable policies.
I agree to comply with all NDTC policies, procedures, terms, and conditions as they exist today and as they may be amended in the future. Continued use of NDTC services constitutes acceptance of such policies and any subsequent revisions.
